Form Filling Instructions
一、本人须在本表首页和末页亲笔签名，并保证所填写信息真实、完整、准确。
The applicants shall personally sign the first and last pages of this form and guarantee that the information filled in is true, complete and accurate.
二、请完整填写申请表（页面不足，可添加附页），并提交所填报信息的相关证明材料（符合报名条件的相关身份、学历、工作经历、任职、专业成果等证明文件复印件），并附近期2寸蓝底免冠照片两张及照片电子版（像素413×626）。
Please complete the application form (extra sheet can be attached for the insufficient pages), and submit relevant supporting materials for the information filled in (copies of relevant identity, academic qualifications, work experience, tenure, professional achievements and other documents that meet the application requirements), together with two recent 2-inch bareheaded photos with a blue background and an electronic version of the photographs (pixels 413*626).
三、担任仲裁员须经申请人所在工作单位填写单位意见并盖章确认。
The appointment of arbitrator shall be confirmed by the applicant’s employer by filling in its opinion and seal.
四、将仲裁员申请表电子版及相关证明材料复印件发送至邮箱tszcwyhzhc2853302@163.com，同时将纸质版申请材料邮寄至唐山仲裁委员会。邮件名称统一为“仲裁员申请表+名字”。
Send the electronic version of the application form and copies of relevant supporting materials to Email tszcwyhzhc2853302@163.com, and mail the paper application materials to Tangshan Arbitration Commission. The name of the mail should be specified as “Application Form for Arbitrators + Name”.
